Our hours are Monday through Thursday, 8:30 am to 12:00 pm and 1:00 pm to 5:00 pm.
Initial appointments require a referral, so please have your physician or chiropractor complete a CONSULTATION REQUEST FORM and FAX or mail it to our address below. Once received, we will call you to set up your appointment. To save time, you can download, print out, and complete our DEMOGRAPHICS FORM and INTAKE FORM ahead of time. Please bring all your completed forms, picture ID and insurance card to the appointment.
